(ii) Are there any form of bacteria present that are made up of more than one cell?

(i) No, bacterial cells do not have well defined nucleus instead, the genetic material in them is dispersed in the cytosol, also there is no internal membrane that delimits a nucleus.
(ii) No, there is no pleuricellular bacteria, i.e., made up of more than one cell. All bacterial cells are unicellular prokaryotes.
